Efek puasa Ramadan pada penderita DM tipe 2 belum sepenuhnya dipahami karena terbatasnya jumlah penelitian yang ada. Sebagian penelitian melaporkan perbaikan parameter metabolik, namun ada pula yang melaporkan terdapat peningkatan kejadian kardiovaskular yang erat kaitannya dengan disfungsi endotel. Tujuan: Mengetahui pengaruh puasa Ramadan terhadap perubahan penanda biologi fungsi endotel pada penderita DM Tipe 2 Metode: Sebuah studi observasional komparatif dilakukan pada bulan April sampai dengan Mei 2019 pada subjek dengan DM tipe 2 yang menjalani Puasa Ramadan. Pada penelitian ini dilakukan pengukuran profil glikemik, profil lipid, parameter antropometri, dan pengukuran penanda biologi fungsi endotel dengan Endotelin-1 (ET-1), Asymmetric dimethylarginine (ADMA), dan Malondialdehidyde (MDA) pada 2-4 minggu sebelum puasa Ramadan (T0) dan setelah setidaknya 14 hari berpuasa (T1). Hasil: Pada penderita DM tipe 2 yang menjalani puasa Ramadan didapatkan penurunan kadar penanda biologi ET-1 yang signifikan (1.83 &#xB1; 0.41 dan 1.44 &#xB1; 0.45 pg/ml, p &lt; 0.001), peningkatan kadar penanda biologi ADMA yang signifikan (231.11 (75.66-571.48) dan 300.38 (96.68-1020.44) ng/ml, p &lt; 0.001), dan tidak didapatkan perbedaan signifikan kadar penanda biologi MDA (0.01 (0.00510.0177) dan 0.01 (0.0049-0.0294) &#xB5;M, p 0.29) pada 2-4 minggu sebelum puasa (T0) dengan setelah setidaknya 14 hari berpuasa (T1). Analisis dengan multilevel linear model memperlihatkan bahwa puasa secara independent mempengaruhi perubahan ADMA dan ET-1. Kesimpulan: Pada penderita DM tipe 2 yang menjalani puasa Ramadan, terjadi perubahan kadar penanda biologi fungsi endotel berupa penurunan kadar ET-1 dan peningkatan kadar ADMA yang signifikan, namun tidak terdapat perubahan signfikan pada kadar MDA. Perubahan ketiga penanda biologi ini belum dapat mencerminkan secara keseluruhan resultan perubahan fungsi endotel yang terjadi pada penderita DM tipe 2 yang menjalani puasa Ramadan. Some studies report improvements in metabolic parameters, but others report an increase in cardiovascular events that is closely related to endothelial dysfunction. Objective: This study aims to show the effect of Ramadan fasting on biomarkers of endothelial function in subject with type 2 DM. Methods: An observational comparative study was conducted from April to Mei 2019 on subjects with type 2 DM who underwent Ramadan Fasting. In this study, an assessment of the glycemic profile, lipid profile, anthropometric parameters, and biomarker measurements of endothelial function with Endothelin-1, ADMA, and MDA were performed in 2-4 weeks before Ramadan fasting (T0) and after at least 14 days of fasting (T1). Results: Type 2 DM patients who underwent Ramadan fasting showed a mean decrease in ET-1 biomarker levels (1.83 &#xB1; 0.41 vs 1.44 &#xB1; 0.45 pg / ml, p &lt; 0.001), an increase in the median level of ADMA biomarkers (231.11 (75.66-571.48) vs 300.38 (96.68) -1020.44) ng / ml, p &lt; 0.001), and there were no median differences in MDA biomarker levels (0.01 (0.0051-0.0177) vs 0.01 (0.0049-0.0294) &#xB5;M, p 0.29) at 2-4 weeks before Ramadan fasting (T0) with after at least 14 fasting (T1). Multilevel linear models analysis showed that fasting independently influences ADMA and ET-1 changes. Conclusion: In patients with type 2 diabetes who undergo Ramadan fasting, there were a significant decrease in ET-1 levels and a significant increase in ADMA levels, but there was no significant change in MDA levels.
