  • Irrational use of drugs is a serious problem in health care because of possible negative impacts that occur. In many countries, at various levels of health services, studies and findings have shown that drug use is far from optimal and rational state. Prescribing drugs play an important role to determine the rationality of the use of drugs, is said to be irrational if the possibility to give little or no benefit at all, while the potential benefits not worth the possible side effects or cost. In this research, the monitoring of outpatient prescriptions Jamkesmas associated with toxic dosing and drug interactions from the Cardiovascular Clinic at General Hospital H. Adam Malik Medan period from January to March 2011. Type of analytic-descriptive study was a prospective cross sectional design. With the data sheet outpatient prescription Jamkesmas who fulfilled the inclusion criteria. Research results showed that from 1019 outpatient prescription sheets of Poly Cardiovascular Jamkesmas in RSUP H. Adam Malik Medan, found that drug interactions occur 28.85% and that do not interact at 71.05%, where the number of the most on prescription by a combination of 5 types of drugs that reach 295 sheets of recipes, then a combination of 6 types of drugs that reach 239 sheets of recipes. Found 294 incidents of drug interactions, consisting of 21 types of drugs that interact, with the mechanism of pharmacokinetic interaction of 34.26%, pharmacodynamics of 51.66% and 14.07% unknown. Level of severity of drug interactions in outpatients include severe levels of interaction 22.04%, 48.73% moderate, and low 29.21%. And there is no toxic dose of outpatient prescriptions of the General Hospital Cardiovascular poly RSUP H. Adam Malik Medan period from January to March 2011.
